Supported Living
A real home, a real tenancy, and the right support to live the life you choose — for adults and for young people moving into adult services.
At a glance:
- Adults & young people in transition
- Complex care & behaviour that challenges
- DoLS, CTOs & Mental Health Act frameworks
- 1:1, 2:1, waking nights & live-in models
- 24-hour support, 365 days a year
The Model
Your own home, with support that flexes around you.
Supported living means living in your own home on your own tenancy — not in a care home — with a personalised package of support to help you live as independently as possible. People keep control of their home, their finances, their relationships and their daily choices.
At Support Health Care Ltd, support can range from a few visiting hours each week through to 24-hour, multi-staffed care. The model is built around the person, working closely with housing providers, families and commissioners.
- Single-occupancy and shared schemes — matched thoughtfully to the people involved.
- Tenancy rights protected — supporting people to understand and exercise their rights.
- Outcomes-focused planning — building skills, confidence and independence over time.
Built for complex and specialist needs
Our supported living teams are experienced across a broad spectrum of needs — including people whose care is delivered under legal frameworks. We provide specialist support for adults and young people living with:

Meals & Nutrition
Help with meal preparation, eating and drinking, and support to stay well-nourished and hydrated.
Domestic & Household Tasks
Support to keep the home clean, safe and comfortable, including light housework and laundry.
Mobility & Moving
Safe support with mobility, transfers and the correct use of equipment to prevent falls and injury.
Companionship & Wellbeing
A friendly presence, conversation and emotional support to reduce isolation and lift wellbeing.
Staffing Models
The right level of support — no more, no less.
We design staffing around assessed need and review it as people grow in confidence and independence.
1:1 Dedicated Support
One-to-one support for people who need consistent, focused care to stay safe and engaged throughout the day.
2:1 Enhanced Support
Two-to-one staffing for people with higher-intensity needs, complex health requirements or significant risk.
Waking & Sleeping Nights
Overnight support — waking nights for active needs, sleep-in cover for reassurance and emergencies.
Live-in & 24-hour Models
Named live-in teams and round-the-clock rotas for people who need a constant, familiar presence at home.
Transition Pathways
Supporting young people into adult services.
Moving from children’s services into adulthood is a significant step. We work alongside young people, families, schools and transitions teams to make the move calm, planned and well-paced — not a sudden change.
Our transition support focuses on continuity: keeping trusted relationships where possible, building life skills early, and shaping a home and support plan that reflects the adult the young person is becoming.
- Early, collaborative transition planning
- Skills-building for independent adult living
- Familiar staff and consistent routines, wherever possible
Our Approach
Person-centred practice, grounded in good governance.
Our supported living services are delivered within recognised legal and professional frameworks — keeping people safe while protecting their rights and choices.
Person-centred & PBS
Support plans co-produced with the person, underpinned by Positive Behaviour Support and active engagement.
Mental Capacity Act
Decisions made in line with the MCA 2005 — supporting choice, and acting in a person's best interests when needed.
Multi-disciplinary working
Close coordination with health, social care, psychology and commissioning colleagues around shared goals.
