The Model

Your own home, with support that flexes around you.

Supported living means living in your own home on your own tenancy — not in a care home — with a personalised package of support to help you live as independently as possible. People keep control of their home, their finances, their relationships and their daily choices.

At Support Health Care Ltd, support can range from a few visiting hours each week through to 24-hour, multi-staffed care. The model is built around the person, working closely with housing providers, families and commissioners.

We design staffing around assessed need and review it as people grow in confidence and independence.

1:1 Dedicated Support

One-to-one support for people who need consistent, focused care to stay safe and engaged throughout the day.

2:1 Enhanced Support

Two-to-one staffing for people with higher-intensity needs, complex health requirements or significant risk.

Waking & Sleeping Nights

Overnight support — waking nights for active needs, sleep-in cover for reassurance and emergencies.

Live-in & 24-hour Models

Named live-in teams and round-the-clock rotas for people who need a constant, familiar presence at home.

Transition Pathways

Supporting young people into adult services.

Moving from children’s services into adulthood is a significant step. We work alongside young people, families, schools and transitions teams to make the move calm, planned and well-paced — not a sudden change.

Our transition support focuses on continuity: keeping trusted relationships where possible, building life skills early, and shaping a home and support plan that reflects the adult the young person is becoming.
